Is permanent makeup right for you?

Permanent makeup procedures can be a natural beauty enhancement for some but some may not be the right candidate for it.

Permanent makeup is not suitable for the following…

-Those under 18 years of age

-Pregnant and nursing

-Viral Infections/Diseases (Even if in remission or undetectable)

-Undergoing Chemotherapy ( Must have had last treatment at one year+ and must provide doctors note)

-Heart Conditions

-Autoimmune Disorders

-Epilepsy

-Using Chemical Exfoliating Products or Treatments (Within 6 Weeks of Procedure)

-Undergoing Acne Treatment or Using Acne Treating Products (Within 6 Weeks of Procedure)

-Used Accutane within past year

-Trichotillomania

-Uncontrolled Diabetes (If managed with a medication, you may be a good candidate)

-Uncontrolled blood pressure (We require a doctors note, if prescribed medication is taken)

-Allergies to Lidocaine

-Blood Clotting Disorders

-People with skin diseases such as psoriasis, eczema, herpes, cold sores and undiagnosed rashes or blisters in the area to be tattooed

-People with allergies to makeup, dyes and other pigments (please inquire about a patch allergy test)

-Active skin cancer in the area to be tattooed

-Those who have healing disorders

-Those who are taking antibiotic and/or steroids within 2 weeks of appointment

-Those with thin and oversensitive skin, on acne medication and or using retinol/retina or clinical strength products

Policies

Atmosphere

Our space is a safe place, free of judgment and criticism. The utmost care, respect, and trust will be given to you. In exchange, we ask for the same.

Payment

The standard deposit to retain new services is $200 unless specified differently. Retainer (deposit) payments are NON-REFUNDABLE. Under no circumstances will a retainer (deposit) be refunded. Retainers and deposits are non-transferable to another person.

The full remaining balance will be collected on the day of your appointment, prior to your procedure. Cash and Credit/Debit cards are the only acceptable means of payment. If you are using a credit or debit card there will be a 9% surcharge added to the transaction. Electronic money-transferring services will not be accepted for any reason. This payment is final and will not be considered for refunds for any reason.

The touch-up fee will NOT be charged on your initial visit unless requested and booked on the day of. The price of this appointment is automatically factored into your total balance, only if the touch-up is booked on the same day as your procedure. If the touch-up is NOT scheduled at the initial appointment or within the 6-12 week time period the touch-up will no longer be included in the price.

Scheduling

If you need to reschedule, 72-hour notice is required. If canceled with proper notice, you will be given ONE opportunity to transfer your deposit to reserve a new appointment. In the event of canceling without rescheduling, the deposit is non-refundable and the appointment will be forfeited. In addition, appointment availability for rescheduled appointments is not guaranteed. All refund requests will NOT be considered nor honored for any reason.

Appointments canceled without proper notice will be charged the full remaining balance to the card on file.

Any appointment running greater than 10 minutes late will automatically be canceled and all paid money will be forfeited, in addition to the card on file being charged the full remaining balance.

Touch-up appointments are required for optimal results. This service is NOT automatically booked. This service must be self-booked by the client online within 6-12 weeks from the original appointment date to avoid additional fees. We recommend booking this appointment when you book your initial appointment to avoid schedule conflicts.

Shelby Barrett, Dr. Dave, Harmony Medispa Lawton, Chosen Cosmetics LLC., nor any affiliates of Chosen Cosmetics offers NO GUARANTEE for permanent makeup results.  

Results will vary from client to client on an individual basis. 

Your skin type, lifestyle, cosmetic regimen, health/well-being, and aftercare of your permanent makeup will contribute to your final results.  

Additional touch-ups may be needed to achieve your desired results.  In the event that you are unhappy with your results, we ask that you notify Shelby Barrett no later than 90 days from your procedure. Together we’ll work to help get you closer to your desired results. Any issues that are brought up beyond 90 days from the last appointment will not be considered and will be billed at the standard touch-up rate.


Frequently asked questions.

1. Does it hurt?

Pain is subjective and varies from person to person. However, we do our best to make sure your procedure is as comfortable as possible with the use of topical medical-grade anesthetics.

2. Will I have to remove or grow my eyebrow hair out?

Growing out your eyebrows is not necessary. In fact, we ask that you come with professionally groomed eyebrows, as each step within your procedure has an allocated time that does not include excessive grooming. We will only remove hair that falls outside of the agreed-upon eyebrow shape. All other hairs will be preserved to help us achieve the most realistic look possible.

3. How do we determine the style and color?

We believe in facial harmony. Therefore we do not use templates or elaborate drawing systems that would result in a cookie-cutter one size fits all design. During your appointment, a special time is set aside to hear your desires. After actively listening to your goals, your shape is hand drawn to fit your anatomy, personality, and lifestyle. The color is mixed to match as closely to your natural hair or permanent makeup desires. Your input in this process is greatly valued.

4. How often will I need touch-ups?

Everyone’s body is different and will heal differently depending on their lifestyle, daily routines, and medication. Typically one touch-up is needed 6-12 weeks from the initial appointment. From there, we recommend refreshing the color annually or biennially for vibrancy. Occasionally due to boldness preference, aftercare, or lifestyle, some clients may need additional touch-ups beyond the initial touch-up. Clients are responsible for these touch-ups unless stated otherwise.

5. Will my brow or lash hair stop growing?

When done correctly, permanent makeup does not interfere with brow hair growth. Regular grooming will still be needed and recommended. Eyelash growth typically isn’t impacted by permanent eyeliner or lash enhancement procedures. However, everyone’s body is different and lash hairs may temporarily shed following a procedure.

6. Do you take insurance, financing, or care credit?

Unfortunately, these options are not available. However, the way our payment structure is built into three payments (the deposit, remaining balance, and touch-up) it mimics a payment plan that makes our procedures more financially feasible.

7. Are you licensed and legal to work in Oklahoma?

Oklahoma has the highest and most rigorous standards for permanent makeup in the nation. We adhere to all state regulations, hold current licenses, and insurance and we travel extensively to seek additional certifications.

8. Is it safe?

We take every measure to provide you with a clean and safe procedure. In addition to every procedure taking place within a medical setting, these procedures are overseen by an Oklahoma-licensed and board-certified physician. Top equipment, materials, pigments, and disposable tools are used in every procedure.
